BSA CLASS 1 & 2 PERSONAL HEALTH AND MEDICAL RECORD

Class 2 form is required of all scouts to participate in Troop outings and summer camp

BSA CLASS 3 PERSONAL HEALTH AND MEDICAL RECORD

Class 3 form is required of all adult leaders over 40 years of age to attend summer camp, and for all scouts and leaders participating in "High-Adventur" activites.
